How to Report a Testing Problem:

1) Describe your problem in a Word Document:

  • Look at the Report A Problem - Sample document
  • Copy the Report A Problem - Template document
  • Complete the template:
    • Decribe your issue
    • Document the steps
    • Include Screen Images where appropriate:
    • Include any identifying information (Voucher #, PO # etc.)
  • Save the document in an easy to locate folder (eg: Windows Documents or Downloads folder) for retreival in Service Now

2) Go to Service Now

3) Please note highlighted fields:

Note: If you don't need to describe your problem with Screen Images then you can use the "Please Describe Your Issue" area, but you are limited to 6 lines.
