Step I - Online Medical History Form
All athletes must read the Introduction Letter outlining the new online medical history form. This step should be completed 24 hours in advance of your medical screening.
New Athletes: To enter your information visit www.swol123.net. The first time you visit the website you will need to enter your Wesleyan email address and click Get Password. All students should use their Wesleyan email address. Check your email for your password and log on to www.swol123.net. Students should complete all demographic information and the medical history questionnaire from under the questionnaire tab (click "yes" or "no" and enter details if "yes").
Returning Athletes: To enter your information visit www.swol123.net. Returning students should sign on with their existing password or reset their password. Students should verify/update demographics and then complete the medical history questionnaire found under the questionnaire tab (click "yes" or "no"and enter details if "yes").
Step II - Medical Clearance Packet (click here)
Please complete all of the information on the Medical Clearance Packet form and bring the completed form with you to your medical screening at the athletic injury care office. Please complete all sections of the form including medical records, sickle cell, ADD/ADHD, pre-participating screening acknowledgement, insurance information, Athletic Injury Care privacy practice, assumption of risk and informed consent and injury, illness and concussion reporting acknowledgement form.
Step III - Report to Medical Screening
All athletes are required to go through medical clearance screening prior to participating in official team practices. Spring sport upperclass athletes should report to the Athletic Injury Care office at the assigned date and time, as scheduled below:
Men's Lacrosse -- Wednesday, January 30 from 9:00-11:30 am
Women's Lacrosse -- Thursday, January 31 from 9:00-11:30 am
Softball -- Monday, February 4 from 9:00-11:30 am
Baseball -- Monday, February 4 from 9:00-11:30 am
The make up date for all sports will be Wednesday, February 6 from 9:00-11:30 am
Important Information About Your Medical Screening
1. Do not work out in the 3 hours prior to your medical clearance time.
2. Bring a PHOTOCOPY of the front and back of your medical insurance card. YOU WILL NOT BE ALLOWED TO GO THROUGH MEDICAL CLEARANCE WITHOUT THIS COPY!
